- Three-Year Couple/Family ($110)
- Three-Year Single ($80)
- Annual Couple/Family ($40 for one calendar year)
- Annual Single ($30 for one calendar year)
- High School/College Student (Free!)
You can either print the form and send along with a check to SAHS/WM, 29 Pearl St. NW, Suite 127, Grand Rapids, MI 49503, or submit it by filling it out and hitting the SUBMIT button below. That will take you to a page where you can pay online using PayPal (which accepts all major credit cards, debit cards, money from a PayPal balance, and PayPal Credit).
